Employee Census - A completed employee census will ensure the prices that are quoted are correct the first time around.
Employer Medical Questionnaire - This is filled out by the employer representative and only includes what he/she knows about their employees (no need to ask employees questions).
Individual Questionnaire - Each employee will need to fill one of these out. They also ensure that the prices that are quoted are correct the first time around. This is only needed for groups under 50 employees.
Employee Benefits Fact Finder - Please complete of our fact finder as much as possible. It will also detail the documents we need.
